Our services, including in-office visits and telehealth (video/phone) visits, are available to patients located in the state of Florida, where our provider is licensed. Eligibility for specific treatments, including medication-assisted treatment (MAT), is determined by a licensed provider following an evaluation.

3. Appointments & Payment

• Self-pay/cash pricing for certain services (e.g., telehealth urgent care and addiction treatment) is listed on the relevant service page and is subject to change.
• Insurance-billed services depend on your specific plan and coverage; verifying benefits is the patient’s responsibility.
• Missed appointments or late cancellations may be subject to a fee, as communicated by our office.

4. Prescriptions

Prescriptions issued through in-office or telehealth visits are sent electronically to the pharmacy of your choice, where permitted by law. Controlled substance prescriptions (including Suboxone) are subject to additional state and federal regulations and may require identity verification, periodic in-person evaluation, or other safeguards as required by law.
